The owner of Wild Sage Therapy is a Registered Social Worker in the Province of Ontario, Canada. The social workers providing services through Wild Sage Therapy are subject to the professional and regulatory requirements applicable to their practice in the Province of Ontario, Canada, including the requirements of the Ontario College of Social Workers and Social Service Workers.
We are committed to protecting the privacy and confidentiality of information entrusted to us.
This Privacy Policy explains how information may be collected, used, disclosed, retained, and protected when you:

Wild Sage Therapy does not knowingly collect personally identifiable information from any person under the age of 13. In the case we discover that a child under 13 has provided us with personal information, we immediately delete this from our servers. If you are a parent or guardian and you are aware that your child has provided us with personal information, please contact us at hello@wildsagetherapy.ca so that we may remove your child’s personal information.
In order to help Wild Sage Therapy maintain and ensure that your personal information is accurate and up to date, you must inform us, without delay, of any change in the information you provided to Wild Sage Therapy.

The Website includes a contact form that allows individuals to contact Wild Sage Therapy regarding counselling and psychotherapy services.

preferred therapist, where applicable;
availability;
how you heard about Wild Sage Therapy; and
any additional information you choose to provide .

respond to your inquiry;

consider your preferred therapist;
assist with therapist matching;
assess availability;
facilitate the process of requesting or arranging services; and
otherwise administer the inquiry.
The additional information section is optional.
Please avoid including detailed or highly sensitive information about your physical or mental health, diagnosis, treatment history, or other sensitive matters in the text field.
If you voluntarily provide information that constitutes personal health information, Wild Sage Therapy will handle that information in accordance with applicable legal and professional requirements, including PHIPA where applicable.
7) Account and Appointment Booking Through Jane App (“the Booking Platform”)
We use Jane App, a clinic management and appointment-booking platform operated by Jane Software Inc. (“the Booking Platform”), to facilitate appointment scheduling and related practice-management functions.
To book free consultation or a therapy appointment, you will be required to create and use an account through the Booking Platform system.
The information associated with account creation and booking a free consultation will include the following:
your name;
your phone number;
email address;
authentication information (username and password);
contact information;
appointment information;
the type of service selected, whether it be Individual Therapy or Couples Therapy; and
your preferred therapist.
When arranging an appointment for therapy, the information required for booking an appointment will include the following:
your name;
email address;
your address;
emergency contact name, phone number, and relationship
name, phone number and email address of referring professional (if any)
pertinent medical information;
goals for therapy;
credit card information, if applicable;
consent to treatment and consent to Wild Sage Therapy’s fees and cancellation policies through the intake form; and
other information deemed necessary for the providing of therapy services to the Client.
Wild Sage Therapy may collect or receive information through the Booking Platform in connection with the provision and administration of counselling and psychotherapy services. Wild Sage Therapy may also upload and provide to the Client various documents, session plans, and notes relating to Client care and treatment through the Booking Platform.
Where information constitutes personal health information under PHIPA, Wild Sage Therapy handles that information in accordance with applicable legal and professional obligations.
The Booking Platform may separately collect and process information necessary to operate and authenticate your Booking Platform account. This includes credit card information, balance information, and receipts. Information held by the Booking Platform for its own purposes is subject to the Booking Platform’s own privacy practices.

8) Personal Information
Wild Sage Therapy will always seek your consent before or at the time that we collect personal information. Wild Sage Therapy may collect information about you from a third party if you have consented to that third party to release your information, for example, for payment or credit card processing. Wild Sage Therapy may also utilize the services of third parties and may receive personal information collected by those third parties in the course of the performance of the Services. Wild Sage Therapy requires all third parties to provide confirmation that they are authorized to release your information.
Wild Sage Therapy is the sole and exclusive owner of the Data and will not sell, share, lease, or otherwise distribute or disclose the Data in any way other than as disclosed by this Policy. Data may be collected through multiple pages on the Website as indicated above.
9) Personal Health Information and PHIPA
Wild Sage Therapy recognizes that personal health information is among the most sensitive types of information.
In connection with the counselling and psychotherapy services provided through Wild Sage Therapy, Wild Sage Therapy may collect, use, disclose, retain, and safeguard information relating to individuals who seek or receive services.
Some of this information may constitute personal health information under PHIPA.
Wild Sage Therapy handles personal health information in accordance with PHIPA and other applicable legal and professional obligations.
Personal health information may be collected, used, disclosed, retained, and disposed of for purposes permitted or required by PHIPA and other applicable law.
These purposes may include the following legitimate needs:
providing counselling and psychotherapy services;
assessing and understanding service needs;
communicating with clients;
scheduling and administering appointments;
maintaining clinical records;
managing and administering the practice;
processing payments;
coordinating care where applicable;
fulfilling legal and professional obligations;
addressing risks to health or safety, where permitted or required by law, such as in the event of a medical emergency; and
other purposes permitted or required by law.
Wild Sage Therapy limits the collection, use, and disclosure of personal health information to what is reasonably necessary for the purposes for which it is collected, used, or disclosed, subject to applicable legal exceptions.
Wild Sage Therapy also ensures every Client has the following rights with respect to their personal health information:
the ability to request corrections be made to their personal health information;
have access to a copy of their personal health information, subject to particular exceptions;
the right to withdraw their consent of personal health information collection, use or disclosure by providing notice; and/or
the right to expressly instruct Wild Sage Therapy to not use or disclose personal health information for health care purposes without consent.

You may withdraw consent, where permitted by applicable law. Withdrawal of consent may affect our ability to provide certain services.

11) Cookies and Similar Technologies
To ensure that the Website is optimized for ease of use, we or our service provider(s) may use Cookies to collect standard information about your language preferences, when you visit the Site, your browser type and version, and other similar information. We or our hosting provider use this information to analyze trends, administer sites, track aggregations of users’ movements, and gather demographic information for aggregate use. IP addresses and other log file information are not connected to personally identifiable information.
Wild Sage Therapy may also collect information from web beacons which indicate whether you visited a certain page on a website, opened a promotional email, or clicked on an online advertisement.
Wild Sage Therapy may collect geographic location from your mobile device or IP number to determine the nearest store or suggest specific product shipping offerings.
This Website is not designed to recognize “Do Not Track” requests from web browsers.
12) Embedded YouTube and Instagram Content
The Website may display or include content from third-party platforms, including:
embedded YouTube videos; and
content or excerpts from Wild Sage Therapy 's Instagram account.
When third-party content is embedded in or displayed through the Website, the relevant third-party provider may collect information about your device, browser, IP address, and interaction with the embedded content.

Wild Sage Therapy reserves the right to charge a reasonable amount to respond to requests to produce transcripts, copies, or transfers of its Data. You will be notified prior to being assessed a charge for such services.

Where PHIPA applies, individuals may also have rights to complain to the Information and Privacy Commissioner of Ontario.

notifying affected individuals where required or appropriate;

22) Changes to This Privacy Policy
We may update this Privacy Policy from time to time to reflect changes to our practices, services, technology, or legal obligations.

23) Governing Law

Failure of Wild Sage Therapy to enforce any of the provisions set out in this Privacy Policy or failure to exercise any option to terminate shall not be construed as a waiver of such provisions and shall not affect the validity of this Privacy Policy or any part thereof, or the right thereafter to enforce each and every provision.
26) Agreement to Terms of Policy
By using the Website and/or providing information to us, you accept and agree to the collection and use of your personal information and personal health information for the purposes described hereinabove.
27) Arbitration
In the event of any dispute relating to or arising out of your use of the Privacy Policy, such dispute shall be settled and determined by a single arbitrator in accordance with the Arbitration Act of Ontario. If you and Wild Sage Therapy are unable to agree on an arbitrator within fifteen (15) days after Wild Sage Therapy is made aware of the dispute, then the parties shall submit the matter to the ADR Institute of Ontario (or is successor), which shall choose the arbitrator. The decision of the arbitrator shall be final and binding without appeal on questions for law or fact or for any reason whatsoever. Costs of the arbitration shall be shared equally between you and Wild Sage Therapy. In the event arbitration is not allowed by law, any other action must be brought only in the courts of Ontario, Canada.
